Generalized Anxiety Disorder 7 (GAD-7) Questionnaire
The GAD-7 is similar to the PHQ-9 and frequently done together. Both of them serve to screen for anxiety and depression respectively, and can quantify it to some degree, but not diagnose anxiety and depression. They are more frequently used in family medicine to screen and know if they should refer a patient to a psychiatrist.
The template calculates the final score, based on the answers.
